此活動由 HK Bio-Rhythm R&D Company Limited （以下簡稱“HK Biorhythm”） 與 Satami International Limited（以下簡稱“Satami”）合作提供和管理。HK Bio-Rhythm 將負責整個活動的細節。推廣期：2022 年 5 月 3 日至 5 月 31 日。

當客戶在推廣期間購買任何 CAREBra 全套產品時，就有資格選擇參加 2022 年母親節推廣活動（以下簡稱“活動”）。

什麼優惠？
成功完成活動後，客戶可通過以下方式選擇享受港幣680元或港幣800元現金回贈：
1. 在促銷期間購買CAREBra；2. 不遲於2022年6月7日按以下要求成功參與本次活動；3. 在接下來的 8 個月內（“監測期”）繼續使用每月一次的 CAREBra 乳房健康監測，即截至香港時間2023 年 1 月 31 日。每月至少需要一次成功的監測並獲得專業報告，即至少八次，客戶會免費獲得一次監測期內八次監測之累計資料的總結報告。4. 成功的參與者於監測期完結翌日起計算七天內或不遲於香港時間2023 年 2 月7 日11：59：59，以較早日期為準，依照 CAREBra App 的現金回贈提示訊息申請現金回贈。5. 普通參加者可申請港幣680元現金回贈。如參加者可於監測期內進一步提交近三個月內的乳房身體檢查報告，例如超聲波、乳房造影等，可申請現金回贈港幣800元。6. 如任何顧客於監測期內，當CAREBra 報告出現高風險而經過香港政府任何合資格機構的相關檢查，不幸首次確診為乳癌，HK Biorhythm 將提供港幣 20,000 元的醫療慰問金（僅限首三名，先到先得。需提供証明文件，包括乳房造影檢查報告、醫生診斷報告或 HK Biorhythm 要求的同等文件）。

 

如何參與？
在推廣期間購買 CAREBra 的客戶將收到一份表格，其中詳細說明了活動的條款和條件（以下簡稱“同意書”）。如果同意參加活動，客戶必須仔細閱讀條款和條件並簽署同意書，於購買翌日起計算七天內或不遲於2022 年 6 月 7 日11:59:59，以較早日期為準，通過 CAREBra App 界面上傳 (i) 購買收據和 (ii) 簽署的同意書，成功上傳所需文件的客戶將有資格成為活動的參與者。參加人數上限為1,000人（相當於1,000件CAREBra），先到先得。如有超額，HK Biorhythm 有絕對權利根據先到先得規則拒絕接受超額客戶的申請。

 

如何申請現金回贈和程序？
1. 成功完成上述要求的參與者。2. 於現金回贈申請限期前七個工作天及最後一個工作天HK Biorhythm將會於CAREBra App進行提示。3. 顧客於現金回贈申請截止日期或之前成功完成現金回贈申請。4. 顧客於45個工作天內收到退還款項。5. 參加人數上限為1,000人（相當於1,000件CAREBra），先到先得。如有超額，HK Biorhythm 有絕對權利根據先到先得規則拒絕接受超額客戶的申請。

 

現金回贈方式
1. HK Biorhythm 應確保參與者符合活動的要求。2.相關回贈將在申請日期或成功完成現金回贈申請後45個工作天內(以較晚者為準)，以銀行轉賬的方式發給成功的參與者，除非HK Biorhythm同意的其他方式。3. HK Biorhythm 將負責所有退貨及現金回贈安排。參與者需聯繫HK Biorhythm申請現金回贈並遵守現金回贈準則。

Terms and Conditions

This Campaign is offered and managed by HK Bio-Rhythm R&D Company Limited (Hereafter "HK Biorhythm") with the cooperation with Satami International Limited (Hereafter "Satami"). HK Biorhythm will be responsible for overall Campaign details. Promotional Period: 3 May - 31 May 2022

Customer who purchases any CAREBra (full set) during the promotional period is eligible to opt in the 2022 Mother's Day Promotion Campaign (hereafter the "Campaign").

 

What is the Offer?

Customer can choose to enjoy cash incentive rebate of either HK$680 or HK$800 upon the successful completion of the Campaign by:

  1. Purchase CAREBra in the promotional period.
  2. Successfully participated in the Campaign as per requirements set out below no later than 7/6/2022 HKT.
  3. Continue to use the CAREBra breast health monitoring on monthly basis in the next coming 8 months period (“Monitoring Period”), i.e., ends on 31/01/2023 HKT. A minimum of one successful monitoring with professional report per month is required, i.e., at least 8 times during the Monitoring Period. Customer will receive a free of charge report which summarises the results of the 8 times monitoring during the Monitoring Period.
  4. Successful participant who apply for cash incentive rebate according to the cash incentive rebate notification via CAREBra App within seven days from the following day of the end day of the Monitoring Period or no later than 11:59:59 on 7/2/2023 HKT, whichever is earlier.
  5. Normal participant can apply for a cash rebate of HK$680. If participant can further submit recent three months body check reports e.g., ultrasound, mammogram, etc. during the Monitoring Period can apply for a cash rebate of HK$800.
  6. If any customer unfortunately is diagnosed with breast cancer for the first time during the Monitoring Period, HK Biorhythm will offer a medical condolence payment of HK$20,000 (for the first three cases only, based on first-come-first-serve basis. Supporting documents including mammography report, doctor's diagnosis report, or equivalent as required by HK Biorhythm shall be provided).

 

How to participate?

Customer who purchases the CAREBra during the promotional period will be given a consent form which specifies the details including the terms and conditions of the Campaign (Hereafter the "Consent").

Customer must read the terms and conditions carefully and sign the Consent if agree to participate in the Campaign and then upload via the CAREBra App interface, (i) the purchase receipt and (ii) the signed Consent within seven days from the day following the purchase date or no later than 11:59:59 on 7 June 2022, whichever is earlier. Customers who successful uploaded the required documents will be qualified as the participants of the Campaign. Maximum number of participants is limited to 1,000 only (equivalent to 1,000 pieces of CAREBra), based on first-come-first-serve basis. In case of excess, HK Biorhythm has the absolute right to reject the excess based on the first-come-first-serve rule.

 

How to Apply for Rebate and procedure?

  1. Participants who successfully completed the requirements as set out above.
  2. HK Biorhythm will notify you in the CAREBra App on i) 7 working days before the rebate deadline and ii) the last working day before the rebate deadline.
  3. Customer successfully applies for a refund before the deadline.
  4. Customer receives refund within 45 working days.
  5. The maximum number of participants is limited to 1,000 only (equivalent to 1,000 pieces of CAREBra), based on first-come-first-serve basis. In case of excess, HK Biorhythm has the absolute right to reject the excess based on the first-come-first-serve rule.

 

Means of Rebate

  1. HK Biorhythm shall ensure the requirements of the Campaign are met by participant.
  2. The relevant amount of rebate will be made to successful participant by means of bank transfer, unless other methods to be agreed by HK Biorhythm, within 45 working days of the application date or the successful completion of rebate application, whichever is later.
  3. HK Biorhythm will be responsible for all returns and cash rebate arrangement. Participants shall contact HK Biorhythm to apply for cash rebate and observe all requirements set herein.

In case of any disputes, HK Biorhythm has the absolute discretion and final decision for any matter about the Campaign.
